A fantastic book including word formation, phrasal verbs, idioms, word collocations, vocabulary and many excercises and tests which are extremely helpful not only to test the gained knowledge, but also to prepare for the CAE (in my case). It's just brilliant! The only thing I really miss is an online app, which would be the ideal up-to-date combination!
